Terminations

To notify Allstate Benefits of an employee termination, please submit the employee's name and termination date to csr-allstate@rwam.com.

Note:

  • No premium credits or refunds are available for terminations that occur in the middle of the month
  • The full month premium is due for that employee upon leaving the company
  • If the employee wishes to keep Critical Illness coverage after termination of employment, the Request to Exercise Portability Privilege form should be completed and sent to csr-allstate@rwam.com
    • The full month premium is due for that employee upon leaving the company
  • To cancel coverage following a termination, the employer must contact Allstate Benefits at csr-allstate@rwam.com or by calling 877-888-7926

Invoice or Billing Questions

Invoices are emailed to the billing contact listed on the Customer Agreement around the 25th of each month. If pre-authorized deductions have been elected, these deductions will take place on the first of every month.

Eligibility of Dependents

Eligible dependents are defined as:

  1. Employee's legal or common law marriage spouse or domestic partner; and
  2. Employee's children and spouse/domestic partner's children

A child is defined as:

  • A natural or adopted dependent belonging to the employee or employee's eligible spouse or domestic partner
  • Must be reliant on the employee financially and meet the following criteria:
    • Is under twenty-one (21) years of age; or
    • Is under twenty-five (25) years of age and in attendance at an institution of higher learning on a full-time basis

After the effective date, any person (except newborns) who becomes an eligible dependent can be added to the certificate if Allstate Benefits is notified within thirty-one (31) days after they become eligible.

A child born to the employee or the spouse or domestic partner of the employee will be eligible for coverage. This coverage begins at birth and benefits will be the same as coverage for any other child insured under this coverage. The employee must contact Allstate Benefits to add the child to their policy.

An adopted child or child pending adoption will be covered as follows:

  1. Coverage is retroactive from the child's birth up to 31 days after the birth date
  2. If adoption proceedings have begun and the employee has temporary custody, coverage is provided from the child's birth up to 31 days after the birth date
  3. Coverage begins from the moment of placement
  4. Coverage will be provided if the employee has custody of the child and required premiums are paid. If an employee gets married and desires coverage for a spouse, the employee must notify the policyholder of the marriage within 31 days

If an employee enters into a domestic common law partnership as defined above and desires coverage for their domestic partner, the employee must notify the policyholder of the domestic partnership within thirty-one (31) days of the date the domestic partnership was formed.

Note:

  • Once Allstate Benefits is notified of the marriage, coverage will be revised to include the spouse and a notification will be sent to the insured with the additional premium due
  • If Allstate Benefits is not notified within thirty-one (31) days of the marriage, then evidence of insurability (EOI) will be required for the spouse, before they are added to the policy



Medical Care Support Services (Medisys and PinnacleCare)

An employee can contact the Medisys Nurse Navigator directly at 1-866-780-2756. The Nurse Navigator will be assigned and is the first point of contact to help guide them through their health care challenge and/or assist with claim form completion.

If a consultation in the U.S. is desired, the Medisys Nurse Navigator will share the employee's medical records file (with their consent) with PinnacleCare.

Then, PinnacleCare will:

  • Work with the employee to coordinate all aspects of U.S. care and treatment support
  • Provide the employee with preferential access to top-ranked Medical Centers of Excellence in the U.S. and the nation's leading physicians and specialists
